2011-2012 Hard Copy Order Form – By Mail If you are a returning voucher program participant, you can now order your vouchers online. Please visit the 2011-2012 Online Voucher Program Webstore page for more information! Steps to Order by Mail: 1. Confirm your program qualifies to participate by visiting the Voucher Program Policies Page 2. Download the hard copy 2011-2012 Voucher Program order form and fill it out completely. (Any order forms missing information will be returned) 3. Double check your math to ensure that the total amount ordered is correct 4. Make a copy of your order form for your records 5. Send in a cheque WITH completed order form to BFL (made payable to The Grocery Foundation) Breakfast for Learning Voucher Program 6. Once your cheque has been received, BFL will process and ship your order as quickly as possible. We will send you an invoice with your vouchers for your records. BFL is unable to release any vouchers until payment has been received.
